However, the long-term use of acid suppressants has significant adverse reactions, especially for patients with refractory gastroesophageal reflux disease(RGERD), whose symptoms are often more severe and the recurrence rate is high; GERD is often accompanied by clinical symptoms such as depression and anxiety, which have a serious impact on the daily life of patients. Traditional Chinese medicine has achieved significant therapeutic effects in treating GERD. Therefore, while utilizing targeted treatment with Western medicine, individualized treatment with traditional Chinese medicine can often be combined to reduce the recurrence rate of GERD, reduce dependence on acid suppressants, and improve clinical efficacy and patient quality of life.   • Objective To investigate changes in complement component 3 (C3) levels in children with sepsis and its correlation with the severity of sepsis and to explore the significance of C3 in predicting mortality in children with sepsis. Methods A retrospective analysis was conducted on 529 children with sepsis who were admitted to the Pediatric Intensive Care Unit in Hunan Children's Hospital between November 2019 and September 2021. The children were categorized into two groups based on their prognosis at day 28 after sepsis diagnosis: the survival group (n=471) and the death group (n=58). Additionally, the children were divided into normal C3 group (n=273) and reduced C3 group (n=256) based on the median C3 level (0.77 g/L) within 24 hours of admission. Clinical data and laboratory markers were compared between the groups, and assess the predictive value of C3 levels in relation to sepsis-related mortality. Results The death group exhibited significantly lower C3 levels compared to the survival group (P<0.05). Multivariate logistic regression analysis revealed that higher pediatric Sequential Organ Failure Assessment (p-SOFA) scores and lower C3 levels were closely associated with sepsis-related mortality (P<0.05). Recent studies have shown that cardiomyocyte lipid toxicity caused by diabetic lipid metabolism disorder may be a key factor in the occurrence of diabetic heart disease. Lipid toxicity can induce myocardial inflammation, oxidative stress, insulin resistance, myocardial fibrosis, microvascular dysfunction, resulting in apoptosis, myocardial fibrosis and diastolic dysfunction, leading to diabetic heart disease, can eventually progress to heart failure, leading to increased mortality in patients. At present, the spe-cific mechanism of cardiomyocyte lipid toxicity leading to the development of diabetic heart disease is not clear. Therefore, this article reviews the role and possible mechanism of lipid toxicity in the pathogenesis of diabetic heart disease, in order to provide reference for basic research and clinical diagnosis and treatment.

Mediating effect analysis was used to analyze the mediating effect of physical activity (PA) on correlation between sedentary leisure-time and body fat percentage (BFP), waist circumference (WC) and body mass index (BMI).Results After adjusted for age, gender, smoking status, alcohol consumption, education levels, intake frequencies of meat and intake frequencies of fresh fruit, sedentary leisure-time (SLT) was negatively correlated with PA ( =-0.246, <0.001), but positively associated with BFP ( =0.061, <0.001), WC ( =0.087, <0.001) and BMI ( =0.071, <0.001). After including the mediator variable PA, the direct effect of SLT on obesity index was still significant. PA was negatively correlated with BFP, WC and BMI ( -0.052, -0.083 and -0.028, respectively, <0.001). Analysis of mediating effect indicated that the association of SLT with BFP, WC and BMI were partly mediated by PA, the proportion of mediating effect was 20.820%, 23.421% and 9.915%.
